Features / Cases
Coordinate support · Cases

One workspace for the whole episode of care.

A case organizes everything for a defined period of support — ownership, services, goals, reviews, and closure. Start as a simple place to log services, and add a formal treatment plan only when the work calls for it.

A services-only case is complete enough to use — goals and a formal plan are there when the work needs them.
Start simple, add structure

As much plan as the work needs — no more.

Level 1
Services-only case
A place to organize and log the support you deliver during an episode of care. Complete on its own.
Level 2
Add case goals
Attach measurable outcomes when tracking progress would help — before or after any formal plan.
Level 3
Formal treatment plan
Add plan context, participants, frequency, and a planned end date — then establish and review it.
Always clear where you stand

Five planning states, one honest picture.

The treatment-plan card always shows the case's current state and the next action available — no guessing whether a plan is "done."

Services-only case
Organizing services without goals or a formal plan. A valid, complete state.
Case goals
One or more goals belong to the case, without a formal plan yet.
Draft treatment plan
Plan context is saved but not yet formally established.
Established treatment plan
The current plan has an establishment date and can be reviewed.
Plan needs review
A reopened established case requiring formal follow-up.
Reviews & closure

A durable record of what happened — and why.

Case reviews
Scheduled, ad hoc, re-establishment, and discharge reviews are recorded as distinct historical events.
Structured closure
Record a closure reason, outcome, and summary — and choose a disposition for each active goal.
Close & reopen safely
Closing ends the episode without erasing history; reopening resumes the same case and flags it for review.
How it works

From first check-in to documented closure.

Open a case, work inside it, and let its timeline preserve the whole story of coordinated support.

1
From a student, open Cases and start a case with its category, owner, priority, and presenting concern.
2
Log services against the case right away — no plan required to begin.
3
Add case goals when measurable outcomes would help, before or after any plan.
4
When the work calls for it, add a treatment plan — context, participants, frequency, end date — and establish it.
5
Record reviews as the plan continues, then close with a reason, outcome, and goal dispositions.
